I accept cash,  check, Visa, Mastercard, American Express, and Discover. There is a $25 fee for  all returned checks. 

The initial session  is $150 for 53+ minutes. Subsequent sessions are $125 for 53+ minutes. Payment is  due at the time of the service. 

If you do not show to your scheduled appointment or provide 24-hour advanced notice, you will be required to pay a cancellation fee.

I am an in-network provider for HealthChoice. I am considered an out of network provider for all other insurances. I can file health insurance claims for you as an out of network provider, however it is your responsibility to know if your insurance has out of network benefits. You may receive reimbursement (as determined by your health insurance plan) directly from your health insurance plan after I have filed them with your insurance. Services may be covered in full or in part by your health insurance. Please be aware that most plans have co-pays and deductibles or may place limits on the number of sessions per year. You are responsible to pay for unmet deductibles and any services rendered that are not paid by your insurance. Payment for services (applicable copays, co-insurance, unmet deductibles, or out of network services) are due at the time of the service.

I encourage you to check coverage with your provider by asking the following  questions:  

Do I have outpatient  mental health benefits? 

How many sessions per  calendar year does my plan cover? 

What is my deductible and  what portion has been met? 

How much is my co-pay,  co-insurance?  

Do I have out of network  benefits? 

Do out of network  services apply towards my deductible?
